You can self refer for physiotherapy in 3 ways:

• Monday to Friday 9:00am to 12:00pm (excluding bank holidays)

Referral Via Form

• You can refer yourself for physio by filling the Self-referral Form

Booking an appointment in Physio Access Clinic:

• This is a face-to-face assessment clinic that will initially be held in the Port Talbot Resource Centre (SA12 7BJ)
