New Patient Registration
If you live in our Practice Boundary Area you can apply to register.
Registration forms are available at Reception.
What you will need:-
- Proof of Address
- NHS Number
- List of regular medication if you have this.
Please fill in all questions on registration paperwork. This will help us help you as best we can whilst we wait for your full medical records to transfer, this can take about 6 weeks.
Once registered, please remember to keep us informed if you change any of your personal details, such as telephone, address, or name. Mobile numbers are important as we send appointment reminders and important information by SMS text, in an emergency it is vital for you and loved ones that we have a means of contacting you. You can do this online.
Temporary Registration
If you are unwell while away from home or if you are not registered with a local doctor you can call 111 for urgent medical advice. 999 in an emergency.
Where needed, you can receive urgent medical treatment from a local GP practice for 14 days.
After 14 days you will need to register as a temporary patient for up to 3 months, or permanent patient.
